Hospital discharge

# After the hospital: planning your parent's next step

Discharge often comes faster than families expect, sometimes with a day's notice. The decisions made in that window shape what Medicare pays for and where your parent recovers.

**3 minute read** Updated October 9, 2026Written by the Senior Choice Care team

In short

Before a parent leaves the hospital, confirm whether they were an inpatient or under observation, because Medicare's nursing home rehab coverage needs a three-day inpatient stay. Then ask the discharge planner which next step fits: home with home health, a skilled nursing facility, or an inpatient rehabilitation facility, and who is arranging it.

## Three questions to ask every day of the stay

1. Is my parent an inpatient or an outpatient under observation? Medicare.gov suggests asking each day, because the answer affects costs and rehab coverage.
2. What will my parent need on the day they leave: therapy, nursing, equipment, someone at home?
3. Who is the discharge planner or case manager, and how do we reach them?

## The usual next steps

^ Where people go after a hospital stay

| Next step | Fits when | Medicare coverage in brief |
|---|---|---|
| --- | --- | --- |
| Home, with home health | Your parent is homebound and needs part-time skilled nursing or therapy | Part A or B, $0 for covered services |
| Skilled nursing facility (SNF) | Daily skilled nursing or therapy that can't be done at home | Up to 100 days after a qualifying three-day inpatient stay |
| Inpatient rehabilitation facility (IRF) | Intensive therapy, physician supervision and coordinated care | Part A, when a provider certifies the need |
| Hospice | Comfort is now the goal and life expectancy is six months or less | Part A hospice benefit |

## Questions families ask

### Does home health need a hospital stay first?

No. Medicare covers home health for people who are homebound and need part-time or intermittent skilled care under a doctor's care, with no prior hospital stay required.

### Who arranges rehab or home health after the hospital?

The hospital's discharge planner or case manager usually makes referrals, but your parent can choose among providers. Check the options on Medicare's Care Compare.

### Does Medicare pay for a nursing home after the hospital?

For short-term skilled care, yes, when the rules are met: a three-day inpatient stay, a Medicare-certified facility, and admission within 30 days. Long-term custodial care is not covered.

### What if my parent can't manage alone at home after discharge?

Raise it before discharge. Ask whether home health, a short rehab stay or extra help at home is needed, and use our [care-level matcher](https://seniorchoicecare.com/assess/care-level-matcher/) to think it through.
