# SNF vs IRF: choosing rehab after a hospital stay

The discharge planner may offer both. They are different kinds of places, with different rules, and the right one depends on how much therapy your parent can handle.

In short

An inpatient rehabilitation facility (IRF) is a hospital-level program for people who need intensive therapy, physician supervision and coordinated care. A skilled nursing facility (SNF) gives daily skilled nursing or therapy at a less intense pace. SNF coverage needs a three-day inpatient stay first; IRF coverage needs a provider to certify the intensive need.

## How to choose between them

- **Can your parent take part in a lot of therapy each day?** If the doctors and therapists think so, ask whether an IRF is an option.
- **What does the hospital team recommend, and why?** Ask the therapist, not only the case manager.
- **What happens after?** Ask how each place plans discharge home and what help your parent will need there.

## The deductible detail

Medicare.gov notes that if your parent goes to an IRF straight from the hospital, or within 60 days of discharge, there is no new deductible, because the hospital stay already started the benefit period.

## Questions families ask

### Is acute rehab the same as an IRF?

Medicare.gov uses inpatient rehabilitation facility and acute care rehabilitation center for the same kind of care, along with rehabilitation hospital.

### Can my parent go to a SNF after an IRF?

Yes, if they still need daily skilled care and meet Medicare's SNF conditions. Ask the IRF team before discharge.

### Which one is better?

Neither is better in general. The right fit depends on how much therapy your parent can handle and what the medical team recommends.
