Decision guide

# When is it time for a nursing home?

Families often wait until a crisis forces the decision. Looking at the signs early gives you time to check homes, sort out payment and involve your parent.

In short

It may be time for a nursing home when your parent needs nursing care every day, needs help around the clock that family and home care can't cover, keeps falling despite help, or when the caregiver can no longer cope. Nursing homes provide nursing care, 24-hour supervision, meals, help with daily activities and therapy.

## Signs to weigh

- Daily skilled needs, such as wound care or complex medicines
- Help needed around the clock, including at night
- Repeated falls or hospital visits, even with help at home
- Dementia with behaviors that are unsafe at home
- A caregiver whose health or job is breaking down
- Home care costs that exceed what a nursing home would cost

## Is assisted living enough instead?

The National Institute on Aging describes assisted living as for people who need help with daily care, but not as much as a nursing home provides. If your parent mainly needs personal care and supervision, assisted living may fit. If daily nursing is needed, a nursing home usually does.

## Before you choose

1. **Name the needs** List what your parent needs now and may need soon. Our [care-level matcher](https://seniorchoicecare.com/assess/care-level-matcher/) helps.
2. **Check the record** Look up homes on Care Compare and read the inspection findings.
3. **Visit** NIA suggests meeting the director and nursing director, and taking Medicare's nursing home checklist.
4. **Sort payment** Medicare doesn't cover long-term custodial care. Ask each home about Medicaid and what it costs.

## Questions families ask

### How do you know when it's time for a nursing home?

When daily nursing or round-the-clock care is needed beyond what home care and family can provide safely, or when safety at home keeps failing.

### Does Medicare pay for a nursing home?

Only short-term skilled care after a qualifying hospital stay. It doesn't pay for long-term custodial care.

### Can a nursing home stay be short term?

Yes. Many people go to a nursing home for short-term rehab after a hospital stay, which Medicare may cover, and then return home.

### What if my parent refuses?

Our guide on [when a parent refuses help](https://seniorchoicecare.com/guides/parent-refuses-help/) has ways to start the conversation.
