Where to find assisted living inspection reports

Medicare doesn't rate assisted living. The record lives with each state, under different names.

Assisted living inspection reports come from the state agency that licenses assisted living, not from Medicare. Use the state's online lookup to confirm the license, see its type, and read recent inspection findings. If a report isn't online, ask the agency or the community for a copy. The table below lists nine states.
