In short
The provider scorecard lets you rate up to three care providers on the same twelve questions: license and inspections, staffing, care planning, communication, cleanliness, costs and fit. Choose Yes, Not sure or No for each. The scorecard adds up totals and counts red flags so you can compare side by side, then print it.
